Choosing a long-term care facility for yourself or your loved ones is a significant challenge. Leaving your home is difficult, and leaving your trusted health providers adds to stress. 

Accordingly, Health & Psychiatry provides integrated behavioral health services to residents of long-term care facilities. Our team approach provides patients with familiar, on-site care and clinical coordination. We offer individualized services and treatment plans for each patient. Our team will coordinate care with patients’ physicians, facility staff, and family members. 

Services are provided on-site throughout Tampa Bay in care units.

For patients in memory care     units, we offer the following     services that include (but are     not limited to): Complete psychiatric evaluations Ongoing medication management Behavioral interventions, psychotherapy,           and psychoeducation Consultation with primary care providers
For facility administrators and     staff, we offer: Behavioral Assessment and Management In-Service Sessions on Behavioral Health On-Call Support for Facility Staff Facilitation of Inpatient Psychiatric           Admissions Collaboration with Primary Care            Physicians, PA’s and ARNP’s	Expert psychiatric consultation related to CMS quality performance indicators such as gradual dose reduction programs, and utilization of psychotropic medications.
